چکیده

Percutaneous transluminal stenting has been well documented in use for the treatment of superior vena cava (SVC) obstruction. It offers relatively safe and rapid alleviation of the symptoms of SVC obstruction due to malignancy compared with other treatment modalities. However, cardiac tamponade after stenting treatment occasionally occurrs. Here, we present a case of 48-year-old female patient with cardiac tamponade after endovascular stenting for malignancy-related SVC syndrome. This patient had facial swelling, headache, dysphagia and dyspnea as the initial presentations. After clinical workup, she had SVC obstruction, due to lung cancer. She received SVC endovascular stenting with an Easy-Wall stent. Three hours after the procedure, she developed cardiac tamponade. An urgent operation confirmed that the protruding pin of the Wall stent had punched through the wall of the SVC and into the aorta, which led to the complication. To avoid this unfortunate result, we suggest that for an obstruction site in the lower half of the SVC, the lower margin of the stent should be placed protruding into the right atrium to get a short free end in the right atrial cavity after stent expansion, so that it will not injury the SVC or the atrial wall after stent expansion. From this case report, we can learn a possible hazard of SVC stenting and propose a technique of stent placement to prevent cardiac tamponade.

Chylous Pericardial Effusion and Tamponade Due to Catheter Induced Superior Vena Caval Obstruction in a Premature Infant

Chylous pericardial effusion and tamponade in children are rare complications of thoracic duct injury during cardiothoracic surgeries or as a result of superior vena cava (SVC) obstruction. We report a rare case of chylous tamponade in a premature infant as a complication of long standing central venous catheterization resulting in SVC obstruction. Urgent pericardiocentesis and then surgical cr...

Cardiac tamponade caused by intrapericardial yolk sac tumor in a boy.

Yolk sac tumor rarely involves the pericardium. A 17-year-old boy had a mediastinal mass leading to total obstruction of the superior vena cava with cardiac tamponade and pleural effusion. Further imaging studies and biopsy showed that the origin was a yolk sac tumor. After surgery and adjuvant chemotherapy, no recurrence was noted during a follow-up of 20 months. This report describes the magn...

Precursor T-Cell Lymphoblastic Lymphoma Presenting as Cardiac Tamponade in a 25-Year-Old Male: A Case Report and Review of Literature

Precursor T-cell lymphoblastic lymphoma (LBL) and T-cell acute lymphoblastic leukemia (ALL) are considered same disease with different clinical presentations. Clinically, a case is defined as lymphoma if there is a mass lesion in the mediastinum or elsewhere and < 25% blasts in the bone marrow. Whereas, bone marrow with > 25% blasts with or without mediastinal masses is classified as T-cell ALL...

Endovascular stenting for end-stage lung cancer patients with superior vena cava syndrome post first-line treatments - A single-center experience and literature review.

BACKGROUND Superior vena cava (SVC) syndrome is a major complication that occurs when a growing lung malignancy compresses the SVC extrinsically. Current treatment options include radiotherapy or chemotherapy to shrink the tumor or endovascular stenting of the SVC to restore flow. Herein, we report a case series treated in a single institution to demonstrate the safety, effectiveness, and outco...

Antithrombotic Therapy Post Endovascular Stenting for Superior Vena Cava Syndrome.

Superior vena cava syndrome results in obstruction of the superior vena cava and commonly requires intervention. Stents are often used to increase the patency of the vena cava and to prevent reocclusion. The role of antithrombotic therapy and agents to use has been highly debated due to stent location, hematologic complications, and limited clinical evidence available on the topic. This comment...
